PDF Forms

PDF Forms lets you upload existing PDF documents (consent forms, assessment tools, NDIS-specific forms) and send them to participants or staff for digital completion and e-signature — no printing required.

Where to find it: Navigate to Forms → PDF Forms in the sidebar.

Uploading a PDF Form

Go to Forms → PDF Forms and click "+ Upload PDF Form".

Give the form a name (e.g. "Service Agreement 2025", "Consent to Share Information") and upload the PDF file.

Use the PDF builder to add fillable fields over the PDF — drag text boxes, signature fields, checkboxes, and date pickers onto the document pages.

Save the form template. It is now ready to be sent.

Sending a PDF Form

From the PDF Forms list, click "Send" next to the form you want to send.

Select who to send it to: a specific participant, a staff member, or enter an email address manually for external recipients (e.g. a participant's guardian).

Add an optional message to accompany the form. Click "Send".

The recipient receives an email with a secure link to fill in and sign the form in their browser — no app or software required.

Tracking Submissions

Each sent form has a status:

StatusMeaning
SentForm delivered to the recipient; awaiting completion
OpenedRecipient has opened the form link but not yet submitted
CompletedRecipient has filled in and submitted the form
ExpiredThe link has expired (default: 30 days) without being completed

Click a submission in the list to view the completed form with all filled-in responses and the e-signature.

Click "Download PDF" to save the completed, signed copy. It is also automatically saved to the participant or staff member's Documents folder.
Use PDF Forms for any document that needs a participant's or guardian's signature — consent to services, consent to photograph, medication authority forms, privacy consent, etc.
