Incidents & Reporting

All NDIS providers are required to record and report incidents involving participants. Plannor's incident module helps you capture events quickly, manage investigations, and meet NDIS Quality & Safeguards Commission obligations.

Mandatory Reporting: Certain incident types (abuse, neglect, unlawful sexual contact, unexpected deaths) must be reported to the NDIS Quality & Safeguards Commission within 24 hours. Plannor helps you document these, but you must also notify the Commission directly.

Recording an Incident

Go to Participants → [Participant Name] → Incidents tab. Or click "+ New Incident" from the main Incidents menu in the sidebar.

Complete the incident report:

WitnessesNames of anyone who witnessed the incident
Evidence/AttachmentsPhotos, body map diagrams, or supporting documents

Click "Submit Report". A notification is sent immediately to the Company Admin.

Corrective Actions

For each incident, you can add corrective actions — specific steps taken to prevent recurrence:

Open the incident and click "+ Add Corrective Action".

Enter the action description, assign it to a staff member, and set a due date.

When complete, the assigned staff member marks the action as done. The incident can only be closed when all corrective actions are complete.

Incident Register

The full incident register is available under Incidents in the sidebar (Company Admin and Coordinators only). Filter by:

Export the register as a CSV or PDF for audits and NDIS Commission reviews.

NDIS Commission Reporting: For reportable incidents, click "Generate Commission Report" inside the incident to produce a pre-formatted document that matches the NDIS Incident Report template.
