Care Plans

A care plan captures everything your support workers need to know before they start a shift — the participant's disability, support needs, communication style, routines, risks, and emergency procedures.

Opening a Participant's Care Plan

Go to Participants in the sidebar and click the participant's name.

Click the "Care Plan" tab on the participant's profile page.

Care Plan Sections

The care plan is divided into the following sections. Click Edit on any section to update it.

Editing the Care Plan

Within the Care Plan tab, click the "Edit" button on the section you want to update.

A rich-text editor opens. Make your changes — you can use bold, lists, and headings to format the content clearly for support workers.

Click "Save Section". The change is saved with a timestamp and your name (for audit trail).
Review Cycle: NDIS best practice recommends reviewing care plans every 12 months or when the participant's needs change. Plannor shows the last-updated date on each section so you can track this.

Exporting the Care Plan as PDF

From the Care Plan tab, click the "Export PDF" button in the top-right corner.

A formatted PDF downloads to your computer. You can print it or email it to family members or allied health providers.
The PDF includes the participant's name, date of birth, NDIS number, and all care plan sections. Your organisation's logo and name are shown in the header.

Support Worker Access

Support workers assigned to a participant can view (but not edit) the care plan from their shift detail screen on the mobile app. This ensures they have the information they need at the point of care.

Sensitive Information: Care plans may contain private health information. Only assign staff to participants whose care plans they need to access. Review access logs under Settings → Audit Log.
