Goals & Goal Tracking
NDIS plans are built around goals. Plannor lets you document each participant's goals from their NDIS plan, track progress over time, and generate outcome reports for plan reviews.
Adding a Goal

Open the participant's profile and click the "Goals" tab.

Click "+ Add Goal".

Fill in the goal details:

| Goal Title | Short title copied from the NDIS plan (e.g. "Improve independent living skills") |
| Goal Description | Full description — what the participant wants to achieve and why it matters to them |
| Goal Category | Daily Activities, Social & Community, Employment, Health & Wellbeing, Learning, or Custom |
| Target Date | When this goal should be achieved by (usually end of NDIS plan period) |
| Success Indicators | Observable, measurable signs that the goal has been achieved |
| Priority | High / Medium / Low |

Click "Save Goal".
Recording Progress
Progress updates can be added directly to a goal or automatically linked from progress notes that reference the goal.

Click on a goal to expand it, then click "+ Add Progress Update".

Enter the update details:

| Date | Date of the observed progress (defaults to today) |
| Progress Level | Slider from 0–100% or choose: Not Started / In Progress / Achieved / Paused |
| Notes | What was observed, what strategies were used, any barriers |
| Recorded By | Auto-fills with the logged-in staff member's name |

Click "Save Update". The goal's progress bar updates immediately.
Linking Goals to Progress Notes
When writing a progress note (after a shift), support workers can tag which goals were addressed during the session. This automatically adds a progress entry to the linked goal and keeps the evidence trail intact for plan reviews.
In the progress note form, expand the "Goals Addressed" section and tick all relevant goals. Each ticked goal gets an automatic progress update with the note content linked.
Goal Progress Report
Generate a summary of all goals and their progress for NDIS plan review meetings.

On the Goals tab, click "Export Goal Report" (top-right).

Choose the date range (typically the full plan period) and click "Generate PDF".

The PDF includes each goal, its current progress percentage, a timeline of updates, and the support worker's notes. Share this with the participant's LAC or planner.
Goal Statuses
| Status | Meaning |
|---|---|
| Not Started | Goal added but no supports delivered toward it yet |
| In Progress | Active work happening; progress updates being recorded |
| Achieved | Goal has been met — mark as achieved and add a completion note |
| Paused | Work on this goal is on hold (e.g. participant is in hospital) |
| Discontinued | Goal is no longer relevant; kept for history but removed from active view |
