Custom Forms
Plannor's Custom Forms builder lets you create multi-step intake forms, assessment questionnaires, and referral forms — and embed them on your website or share via a direct link.
Where to find it: Navigate to Forms → Custom Forms in the sidebar. Available on Professional and above.
Creating a Custom Form
You can create a form manually or use AI to generate one from a prompt.
Option A — Create Manually

Go to Forms → Custom Forms and click "Create Manually".

Give the form a name (e.g. "Participant Intake Form 2025"). This name is internal; participants see the form title you set inside the builder.

In the form builder, add steps (pages) and within each step, add fields. Supported field types:
- Short text, Long text
- Email, Phone, Date
- Single select (dropdown), Multi-select (checkboxes)
- File upload
- Signature pad
- Section heading / description text

Mark required fields, add help text, and set field placeholders. Drag fields to reorder them.

Click "Publish" to make the form live.
Option B — Create with AI

Click "Create with AI" and describe the form you need in plain English (e.g. "A participant intake form asking for personal details, NDIS number, funding type, support needs, emergency contact, and a signature").

The AI generates a complete multi-step form with appropriate fields. Review and adjust in the builder before publishing.
Sharing & Embedding Forms
Each published form has three sharing options:

| Direct Link | Copy the form URL and share via email, SMS, or WhatsApp. Anyone with the link can fill it in. |
| Embed Code | Copy the <iframe> embed code and paste it into your website HTML. The form appears inline on your site. |
| QR Code | Download a QR code for the form — print on brochures or display at reception for walk-in referrals. |
Forms can be embedded on your website's "Referral" or "Enquiry" page. Submissions land directly in Plannor under Forms → Submissions.
Viewing Form Submissions

Click the form name on the Forms list, then click the "Submissions" tab.

Each submission shows the date, the submitter's name (if collected), and all field responses. Click to view the full submission.

From a submission, you can Convert to Participant — Plannor pre-fills a new participant profile with data from the form (name, NDIS number, contact details, etc.).
Form Settings
| Setting | Notes |

| Status | Draft (not visible) or Published (live and accepting submissions) |
| Confirmation Message | Text shown to the submitter after they complete the form |
| Redirect URL | Send submitters to a specific URL (e.g. your website's "thank you" page) after submission |
| Notification Email | Email address to notify when a new submission arrives |
